* The data contained in NPIdentify is taken directly from published data derived from the National Plan & Provider Enumeration System (NPPES) provided monthly by the Centers for Medicare and Medicaid Services (CMS), an agency of the U.S. federal government. Included are provider listings for the 50 U.S. states, District of Columbia and Puerto Rico. No listings showing known armed forces or foreign postal codes are included at this time.
